How to Create a Form from Scratch in SureForms
SureForms lets you create custom forms from scratch without using a pre-built template. You can choose the fields you need, customize the form’s appearance, configure confirmation messages and email notifications, and publish the form when it is ready.
This guide explains how to create a form from scratch in SureForms.
Starting a Form from Scratch
To create a new form:
- From your WordPress dashboard, go to SureForms → New Form.
- On the How do you want to start? screen, select Start from scratch.

SureForms opens the form builder with a blank form.
You can now start building your form by adding the fields you need.
Adding Fields to Your Form
To add a field:
- Open the Add Fields panel in the form builder.
- Select the field you want to add.
- Click the field or drag it into the form builder.
You can add different types of fields depending on the information you want to collect, such as:
- Text
- Number
- Phone
- Dropdown
- Checkbox
- Multi-choice
- Date
- File Upload
- Textarea
- Address
- Other available SureForms fields

After adding a field, click it to open its settings.
Configuring a Form Field
Each field provides settings that allow you to control how it works and appears on the form.
Depending on the field type, you can configure options such as:
- Label: Set the name displayed above the field.
- Required: Make the field mandatory.
- Placeholder: Add example or guidance text inside the field.
- Help Text: Provide additional information to users.
- Default Value: Set a value that appears automatically.
- Advanced: Add custom CSS classes or CSS when supported.

Repeat these steps to add and configure all the fields required for your form.
Reordering and Managing Fields
You can change the order of fields after adding them.
To move a field:
- Select the field you want to move.
- Drag it to the required position.
- Release it when it is in the correct location.
You can also select a field to edit its settings or remove it when it is no longer required.
Customizing the Form Design
After adding your fields, you can customize the appearance of the form.
Depending on the available styling options, you can adjust settings such as:
- Colors
- Field spacing
- Background
- Border radius
Use the form preview to check how your changes look while building the form.

Configuring Form Confirmation
You can configure what users see after submitting the form.
To configure the confirmation behavior:
- Open the form’s Confirmation settings.
- Choose the required confirmation type.
- Configure the success message or redirect settings.
You can display a success message or redirect users to another page or URL after submission.
If you use a success message, customize the heading and message shown after submission.
Configuring Email Notifications
SureForms can send email notifications when a form is submitted.
To configure email notifications:
- Open the Form Settings.

- Click Email Notifications
- Add or edit the notification.
- Enter the recipient email address.
- Configure the email subject and message.
- Add form fields to the email when required.
You can configure notifications for the site administrator and, where supported, send an automatic response to the person who submitted the form.

Publishing Your Form
When your form is ready to accept submissions:
- Review the form and its settings.
- Click Publish at the top-right of the form editor.
- Confirm that the form is published successfully.
Your form is now live and ready to collect submissions.
You can add the form to a page or post using its shortcode or the available SureForms block.

That’s it! You can now create a form from scratch in SureForms, customize it to meet your requirements, and publish it on your website.
